Botox Lip Flip: How Many Units, What It Costs, How Long It Lasts

A lip flip is a very small dose of neuromodulator placed into the upper fibers of the orbicularis oris, the ring of muscle circling the mouth. Relaxing that band reduces the inward pull along the top lip border, so the lip rolls slightly outward and a little more pink vermilion shows at rest and when you smile.

That is the entire mechanism. Nothing is added and nothing fills. The lip is repositioned by a fraction of a millimeter through changed muscle tone, which is why even patients delighted with the result call it subtle, and why it reads better in profile and in motion than in a photograph taken straight on.

It also explains what a lip flip cannot do. It will not build a lip that is genuinely thin or correct asymmetry caused by missing volume, and it does little for the lower lip. It changes how much of the lip you already have is visible, not how much lip there is.

So if the word in your head when you look in the mirror is fuller, the treatment you are picturing is filler. That single distinction is the most useful thing to settle before you book anything.

These two get discussed as alternatives because they are injections in the same place, but they work on opposite principles. One relaxes a muscle. The other places a gel. Booking the wrong one is the most common disappointment in this category, and it is entirely preventable at the consultation.

The practical test is what you want changed. If you want more pink lip showing and a softer upper lip line with no change in size, a flip is a reasonable trial. If you want projection, structure or a measurable increase in size, that is a volume problem and only filler answers it.

One asymmetry is worth knowing before you commit. Hyaluronic acid filler can be dissolved with an enzyme if you dislike it, while a neuromodulator has no reversal agent, so an unwanted flip is waited out rather than corrected. Some practices do combine the two, using filler for size and a small flip to refine how the upper lip sits, which is a legitimate plan when chosen deliberately rather than sold as a package.

The dose is small because the target muscle does a job you use constantly. The orbicularis oris closes and purses the mouth: it shapes consonants, seals your lips around a straw, and makes the tight aperture needed to whistle or play a wind instrument. Relaxing a small fraction of its upper fibers changes how the lip rests. Relaxing more changes how the lip works.

That is why this is the one area where asking for extra units is actively counterproductive. Overtreatment here does not simply look overdone, it shows up functionally: patients describe feeling clumsy forming certain sounds, losing suction on a straw, or finding that an embouchure they have had for years will not seat. Singers, wind players and anyone who presents professionally should say so before a dose is decided.

None of this makes a lip flip dangerous. It makes it precise. Those effects are temporary and dose related, and a conservative number placed accurately sits well inside the margin. But the margin is narrow, which is the honest reason your injector may quote fewer units than you read online.

Most people keep a visible lip flip for roughly 6 to 12 weeks. That is meaningfully shorter than the three to four months commonly seen in the forehead, frown lines or crow's feet, and far shorter than lip filler, usually measured in months to about a year.

Two things drive the difference, and neither means anything went wrong. The first is dose: a few units in a constantly working muscle has less to give than a fuller dose elsewhere. The second is use, since the mouth moves all day for speech, eating and expression, and heavily recruited muscles return sooner.

The practical consequence is scheduling, because a lip flip does not slot neatly into the same calendar as your upper face. Some patients book it separately at a tighter interval, and others time it to land before an event and accept it will fade first.

If duration is what matters most to you, that is itself a signal. Someone who wants a lip change they do not have to think about several times a year is describing a filler outcome, and it is better to hear that at the consultation than after a third short cycle.

Because the unit count is so small, lip flip pricing does not behave like the rest of the neuromodulator menu. Practices handle it in one of three ways, and knowing which you are being quoted explains most of the variation you will see.

Some charge strictly per unit, the most transparent structure, though a minimum charge may quietly set a floor. Some charge a flat fee for the lip flip as a named area, often more than the raw units suggest because it covers consultation, injection time and follow up rather than product alone. And some price it as a discounted add on to a larger neuromodulator appointment, frequently the best value if you were already booking your upper face.

The comparison people most often get wrong is flip against filler. A flip costs a fraction of a syringe at a single visit but lasts a fraction as long, so annualize both before concluding one is cheaper.

Local market, injector credentials, product brand and medical oversight move the number too, which is why no single national figure is worth repeating. Ask for the product name, the units planned, whether a minimum or flat fee applies, and how a two week touch up is handled.

The best candidate has reasonable existing lip volume and a specific complaint about how that lip presents: a top lip that curls inward and disappears when you smile, a border that reads flat, or a wish for slightly more pink with no change in size.

The poor candidate is easier to identify. Anyone whose goal is size, projection or a corrected shape is describing filler, as is anyone with a genuinely thin upper lip, where relaxing the muscle has very little lip to reveal. There is also a functional screen: if your work depends on precise articulation or on a wind or brass embouchure, raise it first, and take an injector who declines as a sign of judgment rather than an obstacle.

The gummy smile overlap is worth separating out. Excess gum show is a different problem, caused by the muscles that elevate the upper lip pulling it higher than you want. It is addressed with a small dose in those elevators, commonly around 2 to 6 units, not by treating the lip border. The two are sometimes done at one visit, but they are distinct targets, so ask which your plan includes.

A lip flip follows the standard neuromodulator arc, so nothing happens immediately. Movement begins changing around three to five days and the fullest effect arrives near ten to fourteen days, which is the first fair moment to judge it. Booking one the day before an event is a common mistake.

Recovery is minimal and there is no downtime in the sense filler has. Small bumps at the injection points usually settle within an hour or two, and bruising is possible because the area carries many small vessels. Injectors commonly advise staying upright for a few hours and avoiding heavy exercise and pressure on the area that day.

The adjustment period is worth preparing for. In the first week or two some people notice the upper lip feels unfamiliar, that certain words take more effort, or that drinking from a bottle feels different. In a well dosed treatment this is mild and settles. If it does not, tell your injector.

Two weeks is also when a decision gets made. If the flip did too little, a very small addition may be appropriate. If it did too much, there is nothing to add and nothing to reverse, so the plan is a lower number next cycle. That asymmetry is why a careful injector starts you low.
