The contact page lists BODi Louisville at 8594 Dixie Highway, Louisville KY 40258, with phone (502) 882-8680 and an SMS line, (502) 673-1515. Google Maps shows the same street address and phone. The practice's second office, at 8201 Shelbyville Road, has separate phone and text numbers.

Each service page has a Book Appointment button that opens an online request form, and the services page says you can also call either office. For Dixie Highway, call (502) 882-8680. The contact page adds an email address, contact@bodilouisville.com, for questions you want to put to the physician.

The banner across the site says all injectable treatments are performed by doctors, and the home page says the same of weight-loss treatment. The staff page lists Suresh Nair MD and Sarita Nair MD, both board certified in internal medicine according to that page, and calls Sarita Nair the Master Injector. Dr Amy Henry also appears on the team.

The weight-loss page lays out three steps: a diet plan, an exercise plan and, if you and your practitioner agree, an appetite suppressant prescription. Monthly follow-up visits are expected, prescriptions are filled at your own pharmacy and cost extra, and the practice does not accept prior authorization requests.

The pages reviewed give no standard treatment prices. A monthly specials page advertises limited-time offers, such as a bring-a-friend Botox discount and a massage and facial package, but those are promotions rather than a price list. Ask the office for current per-unit and per-session prices.

The services menu lists Botox, Dysport and Xeomin. The Botox page explains that all three are brand names for botulinum toxin A, marketed by Allergan, Galderma and Merz respectively, and says Botox is used for lines between the brows, around the eyes and across the forehead.
